Couples Request Appointment - Matrimonios Cita E S P A Ñ O L: Nuestra práctica en Consejería Cristiana se lleva a cabo conforme a las regulaciones profesionales en los estatutos del Estado de la Florida como "Consejería Espiritual Cristiana", por lo tanto con su firma o escibiendo aquimsu nombre, usted declara que acepta que estará recibiendo consejería por video conferencia con un Consejero capacitado en Psicología de Orientación Cristiana. 1. Estas sesiones permanecen bajo absoluta confidencialidad, excepto en el caso en que el consejero evalúe que el aconsejado está en peligro de hacerse daño a sí mismo o a otros, en cuyo caso el consejero podrá involucrar a las autoridades respectivas. 2. Para ingresar a la sesión, el aconsejado proporcionará aquí detalles de la situación que necesita tratar. 3. Dado que estas sesiones son para consejería matrimonial, ambos en el matrimonio (identificado por quien firman esta forma) acepta que estas sesiones se realizan para buscar la resolución de los conflictos matrimoniales y como el consejero trabaja con ambos, se intervendrá para evitar el resultado en divorcio. Si la pareja llega a esa instancia, entienden que este consejero no aceptará intervenir en el proceso judicial de ninguna de las partes ya que ha sido consejero de ambos.4. Dado que las sesiones son confidenciales, tanto el consejero como los aconsejados tienen terminantemente prohibido grabar estas sesiones, salvo que haya existido consentimiento previo escrito y firmado por ambas partes.-------- ENGLISH Our Christian Counseling practice takes place under the professional regulations statutes of the State of Florida as Christian spiritual family counseling, so then by placing your name as your legal signature in this form you acknowledge and accept that the counseling you'll be receiving will be provided to you by video conference and by Christian counselors. 1. If the couple reaches that instance, they understand that this counselor will not accept to intervene in legal proceedings for either party since he has been the counselor to both. 4.Since these are confidential sessions it is prohibited that counselor or counselee record them in any manner or form, unless there is a written and signed approval by both parties.
